After some time I completed this profile trying to fit the functions like a sudoko based on:
- Most used inputs, also for those probable and foreseen in future updates;
- Hotas switches type: 2 way, 3 way, 4 way, momentary or toggle...(it has always bothered me not to understand if the trigger guard was open or close. Now with just one peek i can check the status... and why use a button to close the cockpit door when the joystick paddle looks like to a handle?)
- logical groupings, based both on function and joint use with other inputs;
And, last but not least, based on how I like to enjoy this sim, for example with cold start commands.
Finally, i also dedicated time to the graphic design of the diagram to make it easily readable. I hope it is.

There would still be enough space to assign other keybinds, but I didn't want to create an overflow of inputs just for commands easily manageable out of hotas and/or that can create confusion about their use or their ingame status.
Instead, I tuned the axes of the throttle minijoy ( which in my case do not work properly ) both to be able to move cursor decently and to be able to use the axes as a FLIR Gain/Level concentric knobs.
It takes some practice: with the modifier button pressed, change gain or level and release the modifier button to set.
To modify both, in the same way use the diagonals.
Move and letting the axes self-center then releasing the modifier button will reset setting to default.

Peripheral Rudder recommended (without, it's another game...), but in case you do not have it, replace the Tads FLIR/TV inputs.

To install the profiles:
Start Joy2Vjoy before launch DCS World.
1 - go to Options, then Controls;
2 - In the up/left drop down menu choose "AH-64D Pilot".
Remember to add modifier button: in control main screen choose [MODIFIER]. Then [ADD], you will see "keyboard" by defalut. Open drop down menu and choose Joystick Hotas, then click on the Joystick button used as modifier.  
3 - for each device, in the drop down menu choose "Load Profile..." and load the correct LUA files.
4 - If the Vjoy device is not recognized because it has a different recognition code from mine, enter the commands manually (if needed, following Pellelil tutorial).
Repeat step 2-4 for CPG (adding modifier too) and George.

Even better, even if it seems more inconvenient, i recommend entering all the commands manually following the graphic scheme, then testing and check each one or two times.
This method is faster than it sounds, and allows you to quickly remember commands.

Remember:
To set "Depress fingerlift to release lock" in OPTIONS > SPECIALS > AH-64D - Detent in lockout position,
or it will be easy to risk going in rotor RPM alert.

To assign Hotas TrackIR Recenter button on your "center key" TrackIR software .

To set all Hotas switches in the correct position as mission start status requires (cold start, runaway, airborne...) before fly. It's really simple with just a glance, for how the switches are assigned, as said in the introduction.
