I found this nice Data Card from Easypeac3: https://www.digitalcombatsimulator.com/en/files/3307737/ who in his turn got it from BLMOA https://www.digitalcombatsimulator.com/en/files/3306612/ and converted it to Excel format for personal use (as I find that a bit more easy in use than Photoshop).
Thought I might as well share it here for others to use.

How to use:
- Fill in the Excel sheets as desired
- Select all cells and press ctrl-C
- Open paint and paste with ctrl-V
- Resize paint to 768x1024 (untick maintain aspect ratio, because kneeboards in DCS need to be 768x1024 pixels)
Add to your mission the regular way:
- Open your miz file with winzip
- Add folder named "KNEEBOARD"
- Copy your kneeboard jpeg's in there

Notes
- I know there's a kneeboard generator out there somewhere. I simply never took time to learn how that works and I like to add my own touch to my kneeboards (font style etc.)
- I also added empty 768x1024 jpeg and pdf versions
