Thank you rafalec & PunchOut for letting me know what I had missed.

Corrections to the cockpit text (original now) and squadron text (3rd TFS now 3rd TFW).

69-7555 crashed on take-off returning to its home station, Clark Airbase, after a TDY. The aft section caught fire during take-off. Once airborne, the aircrew ejected successfully. The aircraft crashed into the Yellow Sea off the coast of Kunsan AB, South Korea on 5 April 1982.
